• File a Petition for an Occupational/Restricted Drivers License

Civil and Family Intake

The following are handled in our Civil and Family Intake locations:

• Obtain a Certificate of Name Change
• Obtain an Affidavit of No Record
• Obtain a Certificate of No Appeal
• Obtain an Affidavit of Dormant Judgments
• Release of Surety Bonds posted before July 2003
